Thank you for agreeing to take part in our evaluation. Your response will help us to evaluate and improve the counselling service.

We value all feedback, both positive and negative. It will be helpful if you complete all questions, there are 14 in total and it should take approximately 3 minutes to complete. If you prefer not to answer some of the questions, we would still appreciate your input.
